NHPCA Conference Scholarship Recommendation Form

Scholarship Overview

For qualified persons who wish to attend the 2018 Nebraska Hospice and Palliative Care Association “Living a Good Life...at the End of Life” Annual Conference, NHPCA will award seven one-day conference scholarships to each of the following disciplines: (two) nursing, (one) chaplain, (one) social work, (one) volunteer coordinator, (one) bereavement coordinator, (one) hospice aide.

The NHPCA conference scholarships are funded by the Galen Miller Memorial Education Fund, which was established by NHPCA in memory of Galen Miller, late executive vice president of the National Hospice and Palliative Care Organization. The scholarship fund honors Dr. Miller’s passion for hospice and end-of-life care by providing scholarships to hospice and palliative care professionals in Nebraska.

Scholarship Criteria

Scholarship applicants must:

  1. Be presently employed by an NHPCA member agency or organization.
  2. Have a minimum of two (2) years experience in the hospice/palliative care field.
  3. Use the scholarship funds to attend the 2018 NHPCA Conference scheduled for March 20-21 in Lincoln.
  4. Pledge that they will remain in the hospice/palliative care field for a minimum of one year.


Applications will be reviewed by a selection committee, and will be judged based on:

  1. Commitment to hospice/palliative care.
  2. A recommendation form.
  3. Completeness of their application.

How to Submit a Recommendation Form 

Complete the recommendation form below by Feb. 2, 2018. If an application does not have a corresponding recommendation, it will be marked as incomplete and will not be considered in the judging process. An acceptable reference would be a supervisor, manager or owner of the NHPCA agency/organization where the scholarship applicant is employed. All recommendations will be kept confidential.     

Scholarship winners will be notified via email and will be recognized at the NHPCA Awards Banquet on March 21, 2018.
