2018 NHPCA Shining Star Award

NHPCA is pleased to offer this award category in recognition of an individual, group, or organization that has illustrated outstanding leadership in the promotion of end-of-life services, raised awareness of or educated others on end-of-life issues, developed a new program, and/or developed or completed outstanding work in an end-of-life coalition.

Eligibility Criteria
All nominees must be an NHPCA member in good standing at the time of the nomination. This includes individual members, coalition members, hospice members (and employees), business partner members (and employees), allied health partner members (and employees), and developing hospice members (and employees).

Submission Requirements
Submit the nomination form and a typed summary of no more than 1,000 words indicating why you feel the nominee(s) is/are deserving of recognition. When completing the summary, please refer to the individual as “the nominee(s)” and do not identify organizations or locations. This will assist greatly in the judging process.

Recognition
If the nominee(s)' circumstances permit, he/she/they will be invited to attend the NHPCA Annual Awards Banquet on March 20, 2018, in Lincoln.

Deadline
The nomination form and summary must be submitted no later than Feb. 16, 2018. Email kelseys@nehca.org with questions.
